RE:                   Annual Reminder

 

1.         BLANKET TRAVEL

 

Blanket travel and in-state travel policy is outlined in Business Services & Comptroller’s Office memorandum dated November 10, 1999, Travel Regulation Update.  Blanket travel request forms are no longer required.  Trip authorization will be accomplished upon the department head’s approval of the Form 25 Travel Expense Reimbursement.  Department heads may choose to restrict travel under this policy.  Such restrictions must be written and issued to the departmental employees.  Blanket travel is intended to provide an expeditious method for authorizing frequent travel in the State of Indiana and the Illinois counties of Cook, DuPage, Lake and Will.  Occasional trips within the State of Indiana and the cited Illinois counties should be authorized via the Form 17.  Also, all travel related to professional development activities (conferences, workshops, etc.) must be authorized by Form 17 (not allowed under blanket travel policy).

 

2.         OUTSIDE ACTIVITIES & CONFLICT OF INTEREST DISCLOSURE STATEMENT

 

All requests for permission to engage in outside activities and conflict of interest disclosures approved for 2003-2004 expire at the close of the fiscal year June 30, 2004.  If the activity will continue in 2004-2005, a new President’s Office Form 32A and Executive Memorandum C-1 (if applicable) should be submitted.  Forms should also be submitted for each new outside activity.

 

3.         REPORTS ON SABBATICAL LEAVE OF ABSENCE

 

Staff members who have been on sabbatical leave of absence for part or all of the 2003-2004 year should submit written report on their absence to their department head and dean.
